NCCARE360:A Coordinated Response to Health Equity and Health Determinants in NC

Addressing non-clinical needs or social determinants of health to improve outcomes is critical but often overlooked in providing quality patient care. Social needs complicate clinical care and outcomes leading to higher rates of acute and chronic disease, mental illness and substance abuse. Patients may have the desire to improve their health, but inadequate nutrition or housing resources may present challenges to make the necessary changes for better outcomes. NCCARE360 provides the opportunity for health to all North Carolinians by providing public access to resources and helping health and community-based organizations make electronic referrals, communicate in real time, securely share client information, and track outcomes together. Each of these sessions will examine strategies toward patient-centered high quality care and will address social determinants of health in vulnerable populations.
