Recorded Webinars: Honoring + Documenting: Methods and Communication Tools to Capture Patient’s End-of-Life-Wishes Through use of the NC Medical Orders for Scope of Treatment (MOST)

Robust Advance Care Planning (ACP) discussions help patients understand their goals of care, values, and wishes. By having those wishes documented, healthcare professionals can more easily honor those wishes with respect to end of life care. Yet, there are many challenges to broader implementation of these documents. The healthcare industry and the patient may need to overcome multiple barriers to initiate end-of-life conversations. The goal of this two-session webinar is to address these deficiencies by providing an interdisciplinary team approach for guidance (1) on how the MOST form captures comprehensive guidelines for the patient’s wishes, so healthcare professionals have a better opportunity to understand and honor the patient’s wishes, and (2) initiating robust conversations that many healthcare professionals feel uncomfortable or unequipped to have.
